Two waves of triage absorbed every open field report from production campaigns — multi-tenant isolation properties, AI-execution agent safety, ADR fixture-bindability, deploy preflight scans, the LLM state-dedup rule. New patterns landed for ai-prompt-safety, audit-log, multi-tenant-property-test, and six more. The methodology evolved by absorbing what hurt.
